THE PATHOGENESIS
OF ACNE IS MULTIFACTORIAL

Acne is a chronic inflammatory disease of the pilosebaceous unit, and its pathogenesis includes 4 key factors. Two of these factors, excess sebum production and follicular hyperkeratinization, result in the third factor, Cutibacterium acnes (C acnes) proliferation in the pilosebaceous unit. Together, these 3 factors interact and contribute to the fourth key factor, inflammation, which then leads to the formation of acne lesions.2,3

Understanding the ANDROGEN-SEBUM PATHWAY and its role in acne

Acne pathogenesis begins with androgens stimulating the sebacous glands3-5

During hormonal changes, such as those that occur during puberty, androgens bind to androgen receptors in the sebaceous gland in the mid-dermis layer of the skin, stimulating sebaceous gland activity and sebum production. This androgen-sebum pathway then sets off a cascade of acnegenic factors.3-5

Dihydrotestosterone, or DHT, is a potent androgen that is present in circulation and produced in the skin. DHT binds to androgen receptors within sebocytes.5,6

The skin of FEMALES and MALES with acne vulgaris produces higher levels of DHT and testosterone.4,7

Receptor-bound androgens stimulate sebaceous gland development, sebocyte proliferation, and sebum production in both FEMALES and MALES.4,6,8
